Maneig en fisioteràpia de la síndrome genitourinària de la menopausa en dones amb càncer de mama: Scoping Review

Abstract

INTRODUCCIÓ: La Síndrome Genitourinària de la Menopausa (SGM) induïda pel tractament oncològic és una patologia molt prevalent en dones amb càncer de mama. Engloba alteracions en genitals externs, vagina i tracte urinari inferior, causant molèsties que afecten a la seva vida diària. Degut a les controvèrsies existents sobre la seguretat de les teràpies hormonals en aquesta tipologia de pacient, sorgeix la necessitat de crear evidència sobre intervencions no farmacològiques. PREGUNTA DE REVISIÓ: Quin és l’abordatge en fisioteràpia de la SGM induïda pel tractament oncològic en dones amb càncer de mama per millorar la seva qualitat de vida? OBJECTIU: L’objectiu general d’aquesta revisió és resumir la literatura existent en el tractament de la SGM des de la perspectiva de fisioteràpia en dones amb càncer de mama. METODOLOGIA: Es realitza una Scoping Review de la literatura científica entre el 2017-2022, en Català, Castellà i Anglès, en la que s’incloguin les intervencions de fisioteràpia realitzades en dones amb càncer de mama que sofreixen la SGM. S’utilitzen les bases de dades PubMed, CINAHL plus, Scopus i Cochrane Library. RESULTATS: S’inclouen 12 estudis que compleixen els criteris d’inclusió. Resulten diferents modalitats d’intervenció en l’àmbit de la fisioteràpia, com són l’educació, la fisioteràpia del sòl pelvià i l’ús de dispositius de suport i basats en energia, dirigides al tractament dels símptomes genitourinaris en el CM. CONCLUSIÓ: El tractament no hormonal es considera de primera línia pel maneig de la SGM en dones amb CM, dins d’aquest s’inclouen l’educació, la fisioteràpia del sòl pelvià i els dispositius de suport com les modalitats principals. En canvi, l’ús de les teràpies basades en energia és controvertit i falta evidència al respecte.

INTRODUCTION: Genitourinary Syndrome of Menopause (GSM) induced by cancer treatments is a very prevalent pathology in women with breast cancer. It includes alterations in the external genitals, vagina and lower urinary tract, causing discomfort that affects their daily life. Due to the existing controversies regarding the safety of hormonal therapies in this type of patient, the need arises to create evidence on non-pharmacological interventions. REVIEW QUESTION: What is the approach in physiotherapy of GSM induced by oncological treatment in women with breast cancer to improve their quality of life? OBJECTIVE: The overall objective of this review is to summarize the existing literature on the treatment of GSM from the perspective of physiotherapy in women with breast cancer. METHODS: A Scoping Review of the scientific literature is carried out between 2017-2022, in Catalan, Spanish and English, which includes physiotherapy interventions conducted on women with breast cancer who suffer from GSM. The PubMed, CINAHL plus, Scopus and Cochrane Library databases have been used. RESULTS: 12 studies that meet the inclusion criteria have been included. There are different modalities of intervention in the field of physiotherapy, such as education, physiotherapy of the pelvic floor and the use of support and energy-based devices, aimed at the treatment of genitourinary symptoms in breast cancer. CONCLUSION: Non-hormonal treatment is considered first-line for the management of GSM in women with breast cancer, within which education, pelvic floor physiotherapy and support devices are included as the main modalities. However, the use of energy-based therapies is controversial and lacks evidence in this regard.
