Revisió de l’eficàcia dels diferents tractaments farmacològics del dolor neuropàtic

Objectius: revisar l’evidència disponible respecte del tractament del dolor neuropàtic. Metodologia: atès els treballs fets per altres agències d’avaluació, s’ha limitat la cerca a revisions sistemàtiques o guies de pràctica clínica (GPC) publicades des de l’any 2007, utilitzant per a la cerca tant llenguatge lliure com controlat. Les fonts consultades han estat TripDatabese, PubMed i National Guideline Clearinghouse. Resultats: un total de 3 GPC i 19 documents complien els requisits d’inclusió. Diferents agències d’avaluació valoren com a tractaments d’elecció en dolor neuropàtic antidepressius tricíclics (amitriptilina) o anticonvulsivants (gabapentina o pregabalina). En funció de les dades disponibles en cada cas, es prioritzen de manera diferenciada. La informació aportada per tots els documents més rellevants es presenta de manera incompleta, pel que es fa difícil fer generalitzacions. Conclusions: l’amitriptilina s’ha mostrat eficaç i eficient en el tractament del dolor neuropàtic. També la pregabalina i la gabapentina, però amb un perfil de seguretat millor i un cost més elevat. No hi ha elements que permetin situar la pregabalina o la gabapentina com més eficaces, per la qual cosa no són d’elecció atès el seu cost més elevat.

Objectives: to review the available evidence regarding the treatment of neuropathic pain. Methodology: given the work undertaken by other HTA agencies, the scope was limited to systematic reviews or clinical practice guidelines published since 2007, using both free and controlled language for the search. The sources have been TripDatabese, PubMed, National Guideline Clearinghouse Results: three GPC and 19 documents met the inclusion criteria. Different agencies evaluation value as treatments of choice for neuropathic pain tricyclic antidepressants (amitriptyline) or anticonvulsants (gabapentin or pregabalina). Depending on the data available in each case, different priority. Apordada information for all relevant documents is submitted incomplete, so it is difficult to make generalizations. Conclusions: the amitriptyline has been shown effective and efficient in the treatment of neuropathic pain. Pregabalina and gabapentin also, but with a better security profile and greater cost. No items allow saying that it allows more effective as pregabalina or gabapentin, so are not given their choice of higher cost.
